To add trucks – Attach the truck registration, or lease agreement.  

To remove trucks – Attach bill of sale, or lease termination.  

To add drivers - Attach the driver's license photo and MVR if you have. If your company requires 2 years continusly with CDL, please be sure the license shows the 2 years otherwise attached the prior CDL.  

If you don’t provide the documentation requested the insurance company can’t complete your request. 

By signing electronically, you acknowledge that your signature is legally binding, just like a handwritten signature, and you agree not to challenge its validity in the future. All endorsements are subject to review and approval by the insurance company. If the policy results in a premium increase, a down payment will be required to finalize the endorsement.
